Asking the right questions

 The many differences in the way each company conducts their business makes the development of standard record keeping forms difficult.  Contractors need to develop a methodology and format that best suits their own operation.  Good forms don’t need to be complicated.  As long as accuracy and the necessary detail can still be provided, consider developing forms that allow employees to simply circle or check appropriate categories.

The following checklist provides the basic information the employee or subcontractor should provide for the four basic categories of plowing, walkway clearance, vehicle surface ice control and loader operation.

 Plowing

-         date

-         driver/operator name

-         vehicle number

-         time in / time out

-         operation carried out

-         overall site/weather observations

-         site notes (any conditions impeding or preventing the operator form completing the work as specified.)

 Walkway clearance

-         date

-         driver / crew foreman name

-         crew members (plus time in and time out)

-         time in / time out

-         description of work preformed

o       snow clearance

o       cleanup

o       application of ice melter

o       site inspection

-         ice melter details

o       product used

o       percentage of coverage

o       quantity used or rate of application

 -         overall site/weather observations

-         site notes (any conditions impeding or preventing the operator from completing the work as specified, or conditions preventing the immediate or effective use of materials)

 Vehicle Surface Control

-         date

-         driver/operator name

-         vehicle number

-         time in / time out

-         operation (full salt, spot salt, etc.)

-         quantity of material loaded at source

-         product used (salt, treated salt, pickled sand)

-         material quantity spread (at each site)

-         percentage of surface covered

-         special focus of coverage (i.e. drive aisles, shipping areas, etc.)

-         overall site/weather observations

-         site notes (any conditions impeding or preventing the operator from completing the work as specified, or conditions preventing the immediate or effective use of materials)

-         cross check – total load to total spread

 Loader operation

-         date

-         operator’s name

-         time in / time out

-                  operation performed (plow/stack and pile, removal, etc.)

-                  site notes (any conditions impeding pr preventing the operator from completing the work as specified, or conditions preventing the immediate or effective use of materials
